MSSP
(Multipurpose Senior Services Program)
The primary objective of MSSP is to avoid or delay the inappropriate
placement of persons in nursing facilities, while fostering independent
living in the community.
MSSP provides services to eligible clients and their families to
enable clients to remain in their homes. MSSP case management provides
for client assessment, care planning, service arrangement and client
monitoring. A team of health and social service professionals provides
each client with a complete health and psychosocial assessment to
determine the services needed. These services are routinely provided by
Vendors.
Vendors complete tasks such as basic housekeeping chores, personal
hygiene, personal health care chores, grocery shopping, and
transportation to non-emergency medical appointments.
Certain eligibility criteria must be met in order to receive
services. Eligibility for the program is addressed first at screening,
and confirmed throughout participation in the program.
The eligibility criteria are:
- Certifiable for placement in an Intermediate Care Facility (ICF)
or a Skilled Nursing Facility (SNF);
- Age 65 or older;
- Reside within the Valley area of the County of San Bernardino
- Receive Medi-cal under an appropriate aid code
- Appropriate for case management services.
